Is Our Program the Right Fit?
SFM Solutions Independent Living welcomes referrals from social workers, case managers, discharge planners, behavioral health professionals, and community organizations — as well as direct inquiries from prospective residents and their families.
Who We Serve
Our program is designed for adults who are ready to take meaningful steps toward greater stability and independence.
- Adults ages 18 and older
- Able to live independently with minimal assistance
- Capable of managing their own personal care and medications
- Benefit from a stable, structured, and supportive living environment
- Motivated to engage with life-skills development and community involvement
Admission is based on eligibility, suitability, and available space. Each inquiry is reviewed individually to ensure a good fit for both the prospective resident and our existing community.
